I looked at this image and thought that the iPhone and 3G iPhone both look the same, so physically there must be no difference between them. But as I noticed in the other Gear Live screenshots, the backs are different. The colors of the back are also available in both black and white. One difference that may not be noticeable is that the new 3G iPhone weighs two grams less than the old iPhone.
