I went to customer support on the T-Mobile website and requested it there. The response was that I needed to be a customer for 90 days, and then they would provide the info.
When I called customer service, they gave me a 3-digit code to text message the request for the unlock code (like #MIN# for available/used minutes) and also got the 90-day response to try back later.
